1. مما يتكون الكوليسترول؟

الكوليسترول مادة دهنية توجد في جميع خلايا جسمك، وتُهضم من الأطعمة التي نتناولها، وتُنتَج طبيعيًا أيضًا في الكبد. ويتحد مع البروتينات لتكوين البروتينات الدهنية. والكوليسترول ضروري لوظائف مثل إنتاج الهرمونات والهضم1, 2.

وامتلاك مستوى كوليسترول عالٍ (مستوى كوليسترول كلي 5 مليمول/لتر أو أكثر) يعني وجود كوليسترول أكثر من المعتاد في دمك، وقد يزيد هذا خطر إصابتك بنوبة قلبية وسكتة دماغية من خلال التسبب في تصلب الشرايين أو تراكم اللويحات الدهنية في الشرايين، مما يحدّ من تدفق الدم إلى الأعضاء1,3.

وهناك نوعان:

HDL (البروتين الدهني عالي الكثافة)- يُعرف بالكوليسترول "النافع" لأنه يساعد في إزالة الكوليسترول "الضار" من دمك بنقله إلى الكبد، حيث يُفكَّك ويُزال من جسمك. والمستويات الطبيعية 1 مليمول/لتر أو أكثر.

LDL (البروتين الدهني منخفض الكثافة) أو غير HDL- وهذا هو الكوليسترول "الضار"، وإذا وُجد منه الكثير في مجرى الدم، تتراكم الرواسب الدهنية في الأوعية الدموية مسببة التضيّق. والمستويات الطبيعية هي 4 مليمول/لتر أو أقل.

وفحص الكوليسترول مهم لتحديد المشكلات مبكرًا، وتُقدَّم فحوصات الرعاية الصحية في هيئة الصحة الوطنية البريطانية بما في ذلك ضغط الدم وسكر الدم لمن تجاوزوا 40 عامًا.

وإذا كنت قلقًا بشأن مستويات الكوليسترول لديك، فراجع طبيبك العام واطلب فحص دم للكوليسترول، يُشار إليه أحيانًا بـ"ملامح الدهون"1, 3.

2. ما عوامل خطر ارتفاع الكوليسترول

غذائية

تحتوي البروتينات الحيوانية مثل صفار البيض واللحوم والجبن على الكوليسترول، لكن الأهم أن من المساهمين الرئيسيين في ارتفاع الكوليسترول تناول الكثير من الدهون المشبعة مثل الزيوت وشحم الخنزير والزبدة واللحوم الحمراء والأطعمة المقلية، التي تقلل بعد ذلك قدرة الكبد على إزالة الكوليسترول.

نمط الحياة

التدخين - الإقلاع عن التدخين من أهم عوامل نمط الحياة للمساعدة في تقليل خطر أمراض القلب والسكتة الدماغية وأمراض الجهاز التنفسي والسرطان.

انخفاض مستويات التمارين الرياضية- النشاط البدني المعتدل مثل المشي والسباحة وركوب الدراجات سيحسّن عافيتك ولياقتك العامة وصحة قلبك وأوعيتك الدموية.

ومن المهم اتباع نظام غذائي متوازن صديق للقلب، بدهون أقل - خاصة الدهون المتحولة، وتناول الأطعمة النباتية. وسيساعد تناول كميات كبيرة من الألياف مع الكثير من الفاكهة والخضروات في تقليل خطر الإمساك والمساعدة في خفض الكوليسترول وضغط الدم4.

العمر

من المرجح أن يكون الكوليسترول أعلى مع تقدمنا في السن.

العرق

العرق- الأشخاص من جنوب آسيا أكثر عرضة للخطر.

الوراثة

الرجال أكثر عرضة لارتفاع الكوليسترول. وفرط كوليسترول الدم العائلي حالة وراثية، أي يمكن أن تنتقل في العائلات، وتؤدي إلى ارتفاع الكوليسترول وأمراض القلب والأوعية الدموية1, 2.

3. ما علاج ارتفاع الكوليسترول؟

أحيانًا، إذا لم يكن من الممكن تقليل ارتفاع الكوليسترول بتغييرات النظام الغذائي ونمط الحياة وحدها، قد يوصي طبيبك بأدوية موصوفة لخفض الكوليسترول إلى مستويات آمنة مثل الستاتينات.

الستاتينات أقراص فموية تخفض الكوليسترول في مجرى دمك وتحمي جدران الأوعية الدموية. وهي من أكثر الأدوية الموصوفة شيوعًا في المملكة المتحدة. وإذا رأى طبيبك العام أو طبيب القلب أنك تحتاج إلى ستاتين، فسيجد المناسب لك، بحسب تاريخك الطبي ومستوى الكوليسترول لديك.

وتشمل أنواع الستاتينات:

  • أتورفاستاتين (Lipitor)
  • فلوفاستاتين (Lescol)
  • برافاستاتين (Crestor)

4. ما الفيتامينات المفيدة لارتفاع الكوليسترول؟

دُرست فيتامين B3 (النياسين) وفيتامين D وفيتامين E وفيتامين C، ويُعتقد أنها تساعد في خفض الكوليسترول.

5. هل يمكن للفيتامينات المساعدة في ارتفاع الكوليسترول؟

فيتامين B (النياسين)

النياسين فيتامين من مجموعة B ثبت أنه فعال في رفع مستويات HDL7.

فيتامين C

فيتامين C وُجد أنه يساعد في خفض كوليسترول LDL ومستويات الدهون الثلاثية16.

فيتامين D

فيتامين D ضروري لصحة العظام الجيدة، ويؤدي الكوليسترول دورًا في إنتاج فيتامين D من ضوء الشمس. ولفيتامين D أيضًا دور في الجهاز المناعي وصحة القلب8.

فيتامين E

فيتامين E من أهم مضادات الأكسدة القابلة للذوبان في الدهون، ويقلل لويحات تصلب الشرايين وخطر النوبات القلبية9.

6. كيف تحسّن الفيتامينات مستويات الكوليسترول؟

تساعد مكملات فيتامين D في تقليل الكوليسترول الكلي وLDL والدهون الثلاثية. وقد يكون فيتامين D مفيدًا لمن يعانون من عدم كفاية فيتامين D وارتفاع الكوليسترول ممن هم في خطر مرتفع للإصابة بأمراض القلب والأوعية الدموية10.

ويساعد فيتامين E في الوقاية من الضرر الناجم عن بيروكسيد (تفكك) الدهون.7

وللنياسين خصائص مضادة للتصلب، أي أنه يساعد في الوقاية من تراكم اللويحات في الأوعية الدموية، وبالتالي يساعد في رفع مستويات HDL ("الكوليسترول النافع") وخفض الدهون الثلاثية وغير HDL (الكوليسترول "الضار")11.

7. هل يحتاج الكوليسترول إلى فيتامينات؟

فيتامين D مرتبط بالكوليسترول لأننا نحتاج إلى الكوليسترول في خلايا البشرة لصنع فيتامين D من ضوء الشمس، ويُعالَج فيتامين D لاحقًا في الكبد والكليتين8.

8. لماذا تعمل الفيتامينات لارتفاع الكوليسترول؟

قد يمنع النياسين الجسم من تفكيك الدهون ويساعد في تحليل الدهون الضارة وتقليل تصنيع الدهون الثلاثية في الكبد وإفراز LDL من خلال العمل مباشرة على مستقبلات الخلايا12, 13.

ويحمي فيتامين E سلامة أغشية الخلايا7.

ويبدو أن لفيتامين D تأثيرًا مفيدًا على تقليل مستويات الكوليسترول في الدم وتحسين ملامح الدهون، ويُعتقد أن هذا بسبب تأثيره على تقليل امتصاص الكوليسترول14, 10, 15.

ولا يخفض فيتامين C مستويات الكوليسترول تحديدًا، لكن تشير الأدلة إلى أنه قد يساعد في حماية الشرايين الرئيسية من الضرر والمساعدة في تقليل خطر أمراض القلب والأوعية الدموية16.

9. ما العناصر الغذائية الإضافية التي يمكن أن تحسّن ارتفاع الكوليسترول؟

النظام الغذائي الصحي هو الخطوة الأولى لصحة القلب، لكن هناك كثيرًا من المكملات الغذائية والمنتجات الصحية الطبيعية المتاحة دون وصفة طبية لها فوائد صحية بما في ذلك خصائص خافضة للكوليسترول كما هو موضح في التجارب السريرية17. وهذه أفضل المكملات لتناولها لخفض الكوليسترول.

الألياف القابلة للذوبان والسيليوم

الألياف القابلة للذوبان تساعد في خفض امتصاص الكوليسترول في جسمك، والسيليوم نوع من الألياف مشتق من قشور بذور نبتة Plantago Ovata. ويمكن تناوله كحبة أو كمسحوق يُمزج في المشروبات أو الطعام. وقد يساعد في الإمساك لكن الكثير منه قد يسبب الانتفاخ6, 11.

أوميغا 3

أوميغا-3 ثبت أنها تحسّن مستويات كوليسترول HDL. والأسماك الزيتية ومكملات زيت السمك مصدر جيد للدهون غير المشبعة الصحية، وخاصة أوميغا-3. والتوصيات الغذائية هي تناول حصتين من الأسماك الزيتية أسبوعيًا. وتُحتسب المعلبة أو المجمدة أو الطازجة جميعها، مثل السلمون والسردين والبلشار والتروت والرنجة والماكريل6, 11.

والكتان زهرة زرقاء تُزرع في المناخات المعتدلة، وبذور الكتان وزيته على حد سواء مصادر جيدة لأحماض أوميغا-3 الدهنية.

الفيتوستيرولات والستانولات

الفيتوستيرولات والستانولات (الستيرولات النباتية) شموع مشتقة من النباتات. وتساعد في منع أمعائك من امتصاص الكوليسترول في جسمك. وتوجد طبيعيًا في أطعمة مثل الحبوب الكاملة والمكسرات والفواكه والخضروات. ويضيفها مصنّعو الأغذية الآن إلى بعض الأطعمة مثل المارجرين والزبادي. وهناك أدلة جيدة على فائدة تناول الأطعمة النباتية المحتوية على الفيتوستيرولات والستانولات لخفض مستويات كوليسترول LDL 6,11,19, 29.

بروتين الصويا

قد يقلل تناول التوفو وحليب الصويا وفول الصويا المطهو بالبخار الدهون المشبعة والكوليسترول في نظامك الغذائي. وأبلغت الدراسات عن انخفاض في الكوليسترول الكلي وLDL بعد إضافة بروتين الصويا إلى أنظمة غذائية منخفضة الدهون المشبعة والكوليسترول. وأوصى الخبراء بتناول نحو 25-40 غرامًا من بروتين الصويا يوميًا19.

أرز الخميرة الحمراء

هو منتج خميرة Monascus purpureus التي تُنمّى على الأرز الأبيض. وهو غذاء أساسي شائع الاستهلاك في آسيا واستُخدم في الطب الصيني التقليدي.15 وتتوفر بعض مكملات أرز الخميرة الحمراء لخفض الكوليسترول تحتوي على مادة تُسمى موناكولين K لها تركيب كيميائي مشابه للوفاستاتين30. وهناك تفاوت في الجودة بين المصنّعين، وقد يسبب موناكولين K النوع نفسه من الآثار الجانبية للوفاستاتين (مثل تلف العضلات والكليتين والكبد.) وقد دفع هذا إدارة الغذاء والدواء الأمريكية (FDA) إلى الحكم بأن أي منتجات تحتوي على موناكولين K غير معتمدة ولا يمكن بيعها قانونيًا كمكملات11.

البربرين

البربرين مادة كيميائية مرّة الطعم صفراء اللون توجد في بعض النباتات مثل البرباريس الأوروبي والغولدنسيل والغولدثريد وعنب أوريغون والفيلودندرون وكركم الشجر.

وأظهرت الدراسات أن البربرين قد يساعد في تنظيم استقلاب الغلوكوز والدهون، ولديه القدرة على علاج السكري وارتفاع الكوليسترول20.

الزنجبيل

هناك بعض الأدلة على أن الزنجبيل قد يخفض الكوليسترول الكلي والدهون الثلاثية ويقلل كوليسترول LDL ويعزز كوليسترول HDL6.

الشاي الأخضر

يشير تحليل تلوي من المجلة الأمريكية للتغذية السريرية إلى أن الشاي الأخضر يقلل بشكل كبير الكوليسترول الكلي، بما في ذلك LDL. وقد يساعد شاي الرويبوس العشبي (الشجيرة الحمراء) أيضًا في تحسين ملامح الدهون أو مستويات الكوليسترول في الدم11, 20.

10. ما أفضل وقت لتناول الفيتامينات لارتفاع الكوليسترول؟

يمكن تناول الفيتامينات القابلة للذوبان في الماء مثل النياسين وفيتامين C مع الطعام أو بدونه وفي أي وقت من اليوم. وتعتمد الفيتامينات القابلة للذوبان في الدهون مثل فيتاميني E وD على الدهون للامتصاص السليم، لذا يُوصى عمومًا بتناول المركبات القابلة للذوبان في الدهون مع وجبة تحتوي على بعض الدهون21.

11. ما كمية الفيتامينات والمعادن التي يجب أن أتناولها يوميًا لمستويات كوليسترول صحية؟

جرعة النياسين التي قد يوصي بها الطبيب لارتفاع الكوليسترول هي:

  • 500–3000 ملغ يوميًا عن طريق الفم، مقسمة إلى جرعتين أو ثلاث

والمدخول اليومي الموصى به (RNI) من النياسين هو 14 ملغ يوميًا للنساء و16 ملغ يوميًا للرجال6.

وأدت جرعة 500 ملغ/يوم من فيتامين C لمدة أربعة أسابيع على الأقل إلى انخفاض كبير في مستويات LDL والدهون الثلاثية في الدم لدى المشاركين في الدراسة. ويحتاج البالغون بعمر 19 إلى 64 في المتوسط إلى 40 ملغ من فيتامين C يوميًا22.

وكمية فيتامين E التي تحتاجها يوميًا هي:

  • 4-15 ملغ يوميًا للرجال
  • 3-15 ملغ يوميًا للنساء

وتُعدّ جرعة 50-200 ملغ يوميًا آمنة عمومًا للمكملات22.

والجرعة الموصى بها من فيتامين D هي 10 ميكروغرام من فيتامين D للبالغين. وتُكتب أحيانًا 400 وحدة دولية23.

وكما هو الحال مع جميع الأدوية المتاحة دون وصفة طبية، من الأفضل دائمًا طلب المشورة الطبية والتحدث إلى طبيبك أو مقدم الرعاية الصحية قبل البدء في أي مكملات.

12. هل يمكن أن يسبب نقص الفيتامينات مشكلات الكوليسترول؟

ارتبطت مستويات فيتامين D المنخفضة بارتفاع مستويات الكوليسترول والدهون الثلاثية24.

13. هل الكوليسترول الضار علامة على نقص الفيتامينات؟

هناك علاقة مع نقص فيتامين D، إذ يميل من لديهم مستويات منخفضة من فيتامين D إلى ارتفاع الكوليسترول، لكن لم تثبت علاقة السبب والنتيجة حتى الآن25.

14. ما الفيتامينات التي ينبغي أن أتناولها للوقاية من ارتفاع الكوليسترول؟

هناك أدلة جيدة من المراجعات المنهجية على مكملات فيتامين D لخفض الكوليسترول، خاصة إذا كنت في الفئة عالية الخطورة لنقص فيتامين D10, 8, 22.

وخلال الخريف والشتاء، تحتاج إلى الحصول على فيتامين D من نظامك الغذائي لأن الشمس ليست قوية بما يكفي ليصنع الجسم فيتامين D. وتوصي وزارة الصحة والرعاية الاجتماعية البريطانية بأن يتناول البالغون والأطفال فوق أربع سنوات مكملًا يوميًا22, 10.

ويوصي الأطباء أحيانًا بتناول مكملات النياسين للمرضى المصابين بارتفاع الكوليسترول وزيادة خطر أمراض القلب6, 26, 27.

15. من أين تشتري الفيتامينات لارتفاع الكوليسترول؟

الصيدليات ومحلات السوبر ماركت ومتاجر الفيتامينات والمكملات الإلكترونية مثل WeAreFeel.

16. كم من الوقت تحتاج الفيتامينات لتعمل لارتفاع الكوليسترول؟

يعتمد هذا على عوامل مثل الصحة الحالية للشخص الذي يتناولها، وما إذا كان يتناول أي أدوية موصوفة مثل الستاتينات، والجرعة. فعلى سبيل المثال، خفض العلاج بالنياسين دون أي دواء آخر للكوليسترول الكوليسترول الكلي وLDL وزاد HDL على مدى 16 أسبوعًا. وقد يستغرق الأمر حتى ثلاثة أشهر لرؤية تأثير مكملات الفيتامينات على مستويات الكوليسترول28.

17. الآثار الجانبية المحتملة لتناول الفيتامينات لارتفاع الكوليسترول؟

من المهم دائمًا عدم تجاوز الحد الأقصى للمدخول اليومي الموصى به من مكملات الفيتامينات ما لم يوجّهك الطبيب بذلك.

وقد تسبب الجرعة الزائدة من النياسين أعراضًا سلبية مثل الحكة والغثيان والصداع والقيء والدوار وألم أعلى البطن19, 23.

وقد يسبب تناول كميات كبيرة (أكثر من 1,000 ملغ يوميًا) من فيتامين C ألم المعدة والإسهال. ومن غير المرجح أن يسبب تناول أقل من 1,000 ملغ من مكملات فيتامين C يوميًا أي ضرر22.

وقد يسبب تناول مكملات فيتامين D عالية الجرعة لفترة طويلة تراكم كمية مفرطة من الكالسيوم في الجسم (فرط كالسيوم الدم) مما يضعف العظام ويضر بالكليتين والقلب. لا تتجاوز 100 ميكروغرام (4,000 وحدة دولية) من فيتامين D22.

ولا توجد أدلة كافية لمعرفة ما قد تكون عليه تأثيرات تناول جرعات عالية من مكملات فيتامين E كل يوم. لكنه يُخزَّن في الكبد، لذا إذا كان لدينا الكثير منه، فمن الممكن أن تضعف الجرعات العالية وظائف الكبد22.

Having a high cholesterol level (total cholesterol level of 5mml/L or above) means there is more cholesterol than normal in your blood, and this can increase your risk of heart attack and stroke by causing atherosclerosis or a build-up of fatty plaques in the arteries, restricting the flow of blood to the organs1,3. There are two types: HDL (High-density Lipoprotein)- is known as ‘good’ cholesterol as it helps remove the ‘bad’ cholesterol from your blood by taking it to the liver, where it is broken down and removed from your body. Normal levels are 1mmol/L or above. There is good evidence of the benefit of eating plant-based foods containing Phytosterols and Stanols for lowering LDL cholesterol levels 6,11,19, 29. Soy Protein Eating tofu, soy milk, and steamed soybeans can reduce the saturated fat and cholesterol in your diet. Studies have reported a reduction in total and LDL cholesterol after adding soy protein to diets that are low in saturated fat and cholesterol. Experts have recommended an intake of soy protein of around 25g-40g a day19. Red Yeast Rice Is a product of the yeast Monascus purpureus which is grown on white rice. It is a commonly consumed dietary staple in Asia and has been used in Traditional Chinese Medicine.15 There are some Red Yeast Rice supplements available for lowering cholesterol containing a substance called monacolin K which has a similar chemical makeup to Lovastatin30. There is variability in quality between manufacturers and monacolin K can cause the same type of side effects as Lovastatin (e.g. damage to muscles, kidneys, and liver.) Do not exceed 100 micrograms (4,000 IU) of Vitamin D22. There is not enough evidence to know what the effects might be of taking high doses of Vitamin E supplements each day. However, it is stored in the liver so if we have too much, it is possible high doses may impair liver function22.
